Kindergarten registration for the Monticello Central School District will take place during the month of March by appointment only. You must call the registrar at 845-794-7700, ext. 78905 to make an appointment for one of the dates listed below. Kindergarten screening will also take place at the time of registration. Please be sure your child is present.
In order for a child to qualify for the Fall 2023 kindergarten your child must turn 5 years of age on or before December 1, 2023. We have two elementary schools. (Emma C. Chase & George L. Cooke) The school your child will attend is determined by your home address.
Students who will attend the George L. Cooke Elementary School will register at the St. John Street Community School, located at 22 St. John Street in the village of Monticello. Appointments are available on the following dates:
- March 20
- March 21
- March 23
- March 24
Students who will attend the Emma C. Chase Elementary School, will register at the Chase school, located at 28 Pennsylvania Ave in Wurtsboro. Appointments will be held on March 22.
Documents you need to bring at the time or registration:
- Child’s Birth Certificate
- Child’s most recent Immunization Record and physical
- Proof of Residence (utility bill, lease, tax bill, paycheck stub, library card, etc.)**
- Parent’s picture ID (driver’s license or Medicaid benefit card or passport)
- Custody Papers, if applicable
**If you are living with someone else and have no proof of residence in your own name, then we require a notarized affidavit from the person with whom you live stating they give you housing. (Form can be downloaded from the online pre-registration site) Any one of the forms of Proof of Residence listed above must be attached to the notarized affidavit.

Please pre-register online and be sure to bring the required documents, along with the attached health and language questionnaire to your appointment. Completing the online pre-registration ahead of time will expedite the registration process. If you do not have access to a computer, access will be provided to you at the time of your appointment. Please allow approximately 60-70 minutes to complete the registration and screening.
