Q: What do I do if I need a copy of my transcript?

A: Please contact Ms. Yewchuk by phone at (845) 794-8840, ext. 10951 or email jyewchuk@k12mcsd.net to obtain a copy of your high school transcript. You will need to know the year you graduated, what name is on the transcript (it may be your maiden name) and the address of the place where you would like the transcript sent.

Q: What do I do if I need a copy of my immunization records?

A: Please contact the school nurse’s office at 845-794-8840 ext. 10940. You will be asked your date of birth.

Q: How do I get work for my child who is out sick?

A: Please contact the guidance office at 845-794-8840, ext. 10951 and work will be requested for your child.

Q: How do I schedule a parent conference concerning my child with teachers?

A: If you need a meeting with an individual teacher please feel free to contact that teacher via email or phone and set up a meeting when convenient for both of you. If you would like to meet with several or all of your child’s teachers please call your child’s guidance counselor and they will be happy to schedule a parent-teacher conference for you.

Q: I need to get working papers, where do I go?

A: See Ms. Yewchuk in the guidance office to obtain working papers.

Q: I need an attendance verification letter for my child. Where do I go for this?

A: Please contact Ms. Yewchuk in the guidance office for an attendance verification letter for outside agencies.
