The following is a list of Health Screenings and Immunizations that are required for students as of September 2007.

Any questions should be directed to the Department of Pupil Personnel Services at 794-0128.  

HEALTH ISSUES

2007 New York State mandated health screenings are as follows:

  • Monticello High School: September 25-28

  • RJK Middle School: October 2-4

  • Rutherford Elementary School: October 10-12

  • Cooke Elementary School: October 16-18

  • Duggan Elementary School: October 24-26

  • Chase Elementary School: through October

Annual screenings
These additional health screenings are held throughout the year and are provided by New York State law:

  • Physical exam: Grades K, 2, 4, 7, 10 and all new students

  • Vision: Grades K-3, 5, 7, 10 and all new students

  • Hearing: Grades K, 1, 3, 5, 7, 10 and all new students

  • Scoliosis: All students Grades 5-9

Immunizations
New York State mandates the following:

  • Polio: Three-plus doses of oral or four or more doses injectable

  • Measles: Two doses after age 1

  • Mumps: One dose after age 1

  • Rubella: One dose after age 1

  • Diphtheria: Three-plus doses

  • Hepatitis B: Three doses

  • Varicella: One dose for students who entered kindergarten on or after September 1998

  • Tdap (Tetanus-diphtheria-pertussis): One dose for students entering Grades 6 in September 2007, or later
