PRASAD Children's Dental Health Program

Is your child eligible to receive services on PRASAD-CDHP’s Mobile Dental Clinic?

The answer is "Yes," if your child has no dentist, AND is insured by Medicaid, Delta Dental, Empire BCBS, GHI, Wellcare, Fidelis, MVP, Affinity or Hudson Health Plan.

Medical history forms are sent home with all children before the clinic’s visit in order to allow enough time for the application process to be completed by enrolling with the dental insurances. Parents must sign and return the medical history form showing consent for treatment before a child can be seen in the clinic.

Mobile Dental Clinic Schedule: The dental clinic visits Sullivan County schools on a rotating basis. We provided more than 1600 dental visits last year. If your child receives services of PRASAD dental clinic, it is possible to arrange an appointment, even when the clinic is not at your local school site or during the school holidays.
