1. All families should fill out
a form and return it to school the first week of school.
2. All forms must be filled out completely; income, address, Social
Security number, children’s names, grade and school, and all people
living in the household and income (where applicable). If you are on
public assistance, your Food Stamp case number or TANF/FDPIR case
number is required.
3. All children who were on F/R lunch last year will be carried
until September 30, 2007. You must fill out a new form every year.
If we do not have a new form by September 30, 2007 your child
will be required to pay full price.
All children were sent home with a form the first week of school.
Only one form with all children listed is required per family. If
you have not received a form or have questions please call Kathy
Kelley at 794-8840 Ext. 10970.
